Brian serves as the Director of the Governor's Chronic Care Initiative, a practice transformation project that brings together physician office, insurance companies and state government to redesign the way patient care is delivered in primary care settings. With over 160 practices currently participating in 7 regional based collaboratives touching the lives of 1.2 million patients, Pennsylvania has the largest and most unique Chronic Care and Patient Centered Medical Home project in the country. Brian is a graduate of Ursinus College and prior to joining the Governor's Office of Health Care Reform, he served as Legislative, Policy and Special Advisor for the Secretary of the Pennsylvania Department of Health.
